There are often geographical and socioeconomic factors that affect care therefore this survey does contain some questions about where you live and your educational background. There are no risks associated with taking this survey and you may not personally see a benefit. This introduction is provided to you because we will be using this information in the aggregate, this means we will use the totals and averages collected to issue reports and publications about the nature of HCM and how it affects individuals.
