4-H SET County Implementation Survey

Dear 4-H County Staff:

With the support of the Noyce Foundation, National 4-H Council has contracted with Policy Studies Associates (PSA) to evaluate the implementation of the 4-H Science, Engineering, and Technology (SET) initiative.  As part of this evaluation, PSA is surveying county-level staff who are responsible for overseeing the implementation of 4-H SET programs.

The survey is designed to capture basic information about your progress in the early stages of implementation of 4-H SET Programming.  The information you share will be kept confidential and will be used in the aggregate to help improve upon and guide 4-H SET programming, nationwide.  No names of individuals or counties will be used in any reports. 

If you have no SET programs operating in your county, you will have the option to report this in the first three questions and skip the remainder of the survey. Please be sure to respond to the first three questions, however, so that we have a response logged from your county and do not continue to follow-up with you.

Once you have started the survey you can simply close out of your browser when you need to stop; the questions you have already answered will be saved automatically.  You can restart the survey at any time, provided you work from the same computer.  The survey will not be submitted until you reach the end and click the submit button; until you click submit, you may page through the survey and edit your responses to any of the questions.
